GENERAL MONITORING IN INTENSIVE CARE

Karel Zitko

The goal of monitoring is to reach normal values:
- pH in wide range 7,30 - 7,50
- serum sodium 138-145 mmol/l
- serum potassium around 4,0 mmol/l
- serum glocose under 6 mmol/l
- hematocrit 0,30
- serum hemoglobin 10 g/l
- serum albumin 30 g/l
- serum osmolality to 320 mOsm/kg
- serum oncotic pressure >15 mmHg
- diuresis per one hour 50-100 ml/hour
- respiratory rate in spontaneous ventilation < 30 breaths/min.
- inspiration pressure < 30 cmH2O
- systolic blood pressure >100 mmHg
- mean arterial pressure >90 mmHg
- central venous pressure about 10 cm H2O
- SaO2 >95%
- PaO2 >100 mmHg (13 kPa)
- PaCO2 + PetCO2 no low then 30 mmHg (4 kPa)
- ECG is without dysrythmias, without oscillations of pulse rate and without ST segment depression
- body temperature keep best up to 37oC (in cooling as well)
- fluid balance per os and parenterally well - balanced
- early nutrition for keeping anabolic processes (that means about 20-30 kcal/kg/day = 80-125 kJ/kg/day).
